Captain 20 confirmed entrapment due to power lines down around the vehicle and a West Penn Power pole across the hood of the vehicle that was perched on top of the guide rail. Assistant 20-1 took command of the incident and immediately requested West Penn Power along with Comcast and Windstream to the scene. While waiting for the utility companies, contact was made with the single operator of the vehicle after she was told to call 911 so that we could have her cell phone number. The patient was told to remain in the vehicle until the power was shut off, the vehicle was property stabilized, and someone told her that it was safe to exit the vehicle. She reported having minor burns to her hands from airbag deployment but other than that was not injured. The power company took approximately an hour and a half to get to the scene and to confirm that all three phases of electric were off and would not recycle. In that time frame, crews from Rescue 20, Rescue 64, and Murrysville Medic One implemented a plan of extrication that included a variety of cribbing, stabilization struts, ratchet straps, and bumper jacks. With the utilities safely secured, the extrication of the patient from the vehicle commenced. Crews used two stabilization struts on the passenger front side of the vehicle and two box cribs - one on the passenger side rear tire and the other on the driver side between the front and rear tires. The patient was then helped out of the passenger side of the vehicle. Total time for extrication was fifteen minutes once the power was confirmed to be off. After the extrication, crews began to wrap up and waited for Wally's Transport to arrive for the removal of the vehicle from the scene. Good teamwork by all involved! Total time out - four hours. Chief 64 arrived and reported two vehicles, head-on type collission, with one patient trapped in a vehicle. As Rescue 20 approached, Director 610 requested a roof flap and removal of the driver side of the vehicle. Crews from Rescues 20, 64, 78, Engine 20, and Murrysville Medic One went to work using the Hurst spreaders, cutters, and rams. The roof of the vehicle was completley removed to allow the crews from 617 and 613 to handle patient care. The driver side was then removed and the dash was partially rolled to free the patient. The crews worked well together allowing for a quick extrication. With the patient in the care of Murrysville Medic One, crews worked to contain fluids and remove debris from the roadway while Murrysville PD completed their accident investigation. Both vehicles were removed from the scene by McElhinney's Service. Shortly after the call was dispatched, Chief 78 arrived and reported a working house fire with heavy smoke showing from the Delta side of the roof. Engine 22 was first on scene taking the address and stretched an attack line to the interior. Attack 64 picked up the hydrant at Grant and Berlin Farm and supplied Engine 22. The fire was determined to have started in the basement and had actively progressed through the interior walls, floors, and attic. Crews from Ladder 87, Truck 20, Engine and SOC 78 opened up walls and ceilings on the first and second divisions while trying to control the deep seated fire. The fire was marked under control within an hour and a half. Salvage 63 handled the remaining salvage work and mop up. Companies were released as they picked up. Medic One provided medical and rehab services. Murrysville PD provided traffic control shutting down Berlin Farm Road. The PSP Fire Marshal is currently investigating the cause and origin of the fire that displaced two adults and seven children. Upon acknowledgement of the call, WCDPS advised of one male patient down on the roadway that was unconscious but breathing. Rescue 20 went enroute with a crew of four, and was advised by Director 610 to take Old William Penn to Cline Hollow and to proceed westbound in the eastbound lanes of Route 22. Rescue 20 arrived and setup a landing zone for an inbound LifeFlight helicopter. Engine 20 shut down westbound Route 22 while awaiting for the helicopter to land. The patient was then packaged and transported to AGH via LifeFlight. With the helicopter safely off the ground, crews began traffic control operations. Engine 22 was dispatched to assist and was sent to Route 22 at Tarr Hollow Road to direct all eastbound traffic on to Old William Penn Highway. The traffic coming eastbound that was waiting between Sheetz and Tarr Hollow was cleared by using the westbound lanes. Rescue 20 then provided scene lighting for the PA State Police and Murrysville Police while they conducting R & I. Crews were out approximately three hours. Unfortunately, the patient that was struck passed away early the next morning at AGH. The show featured fire equipment from Sutphen, Precision Fire Apparatus and Alexis Fire Equipment. Many aerials, pumpers, tankers and rescue trucks arrived in the morning to be on display. There was a lot of shop talk between the fire department members viewing the equipment. A special thanks to Chief Mark Piantine of the Bradenville Vol. Fire Co. for having a great lunch. Chief 20 notified of gas leak by phone prior to call of a gas valve left open in one of the classrooms in the Science wing. The Chief requested the first alarm along with two additional vent trucks. Before the first apparatus arrived, all participants in a volleyball tournament were safely evacuated from the building. Truck 20 arrived on scene and sent a crew to the interior with the MSA Sirius meter. The custodial staff had already opened windows in all classrooms on the first and second floor of the structure and were utilizing a fan to ventilate the classroom where the open valve was found. No readings were found, and at this time the alarm was scaled down to Truck 20, Special Operations 78, and Vent 112. Vent 112 operated from the Charlie side of the building ventilating for approximately a half hour. This was done as a precautionary measure as there were no readings of explosive gas but a residual gas odor. With ventilation complete, crews took final readings in the building confirming the building was safe for people to occupy. Crews were out approximately 2 hours. Sat, 03 Sep 2011 23:06:41 GMT http://www.murrysvillefire.com/apps/public/news/newsView.cfm?News_ID=289 Ammonia Spill at Murrysville Business Park http://www.murrysvillefire.com/apps/public/news/newsView.cfm?News_ID=288 At 14:57 hours, Truck 20 was dispatched to 101 Technology Drive in the Murrysville Business Park at General Maintenance Products & Supply, Inc. for an ammonia spill inside the building with patients. With annual hose testing going on at the station, Truck 20 went enroute quickly with a crew of five. The truck arrived shortly behind Engine 22 and staged on Side A of the building to prepare for emergency decontamination of the patients if necessary. With all patients being accounted for and no need for emergency decontamination, all crews staged to await the arrival of Team 800 (Westmoreland Hazardous Materials Response Team). During this time, Murrysville Medic One transported two of the seven employees that had been inside when the spill occurred. Once Team 800 arrived, an entry team was sent inside the building to assess the situation. There was 20 to 30 gallons of aqueous ammonia that had spilled out of a 55 gallon drum. The product had ran in various directions and was underneath pallets of other materials. While the entry team was inside the structure, the crew from Truck 20 helped assemble the decontamination area for the entry team. Every time an entry team exited the building they came through decontamination to ensure that there was no product left on their protective suits. Entry team #1 and #2 decontamination was handled by four operators from T20. Entry team #3 decontamination was handled by 78, 63, and 22. A total of three entry teams (made up of two Team members each) worked inside of the structure using absorb-all to soak up the spilled product. The product was then transferred to a bung. When the spilled product had finally been safely absorbed and moved to a container, the vent trucks from Jeannette (Vent 112) and Westmoreland City (Vent 3) began to circulate air through the building to ventilate the dangerous fumes. Once the fans had run for an appropriate amount of time, members from Team 800 went back inside to monitor the air. Eventually the air inside the structure was deemed safe. Crews were out a total of five and a half hours. Good job to all of those on scene who worked through a lengthy incident! Truck 20 went enroute with a full crew followed by Rescue 20 with a crew of four. Chief 64 was the first to arrive on scene and reported a working fire with live power lines down across the roadway. Attack 64 picked up the hydrant at Monticello Drive and laid in to the scene. Engine 30-1 picked up the other hydrant near the structure and would later supply Ladder 87. Crews from Attack 64 and Engine 22 pulled multiple attack lines to begin knocking down the fire. Truck 20 arrived and was instructed to setup on Side D of the structure. The master streams from both Truck 20 and Ladder 87 were deployed along with multiple handlines. Crews flowed water for well over an hour before the bulk of the fire was knocked down. A lengthy overhaul process then began involving crews from multiple companies. The crew from Truck 20 and Engine 78 worked the remaining stubborn fire between different sections of roof while the State Police fire marshal began their investigation. Assistant 236 on scene with smoke showing and a bedroom well off in a two story single family residential. The Engine crew from 236 advanced a hose line to the second story of the structure and made quick work of the room and contents fire. The crew from T20 and R20 set ground ladders, placed salvage covers, and searched for a few pets that were unaccounted for. Crews from Sardis, Renton, and Holiday Park secured a water supply, secured the utilities, and provided hands for overhaul and ventilation. The scene was turned over to the Allegheny County Fire Marshall. Total time out was one and a half hours. Good work by all on a humid summer evening! With another call pending in Level Green, the crews split up with R20 proceeding to the house struck by lightning on Antler Hill Drive and T20 heading to Salem Township. T20 was staged on Side A of the manufacturing facility and was instructed to send their personnel to Side C staging. Crews remained in staging while a plan was formulated on how to deal with the machine that was on fire. Fire extinguishers were used to knock down the molten plastic that had been pressed out of the mold. The maintenance manager confirmed that the machine had cooled (as it was supposed to do) to 41 degrees celsius (106 degrees F) and that it needed an additional 20 minutes to cool. Shortly after 19:00 hours, the press was opened and the fire was confirmed out. The cause of the fire was due to a power outage that caused the machine to restart its program when the power came back on. The plastic slab in the mold was already up to temperature and the program applied too much pressure to the material causing it to come out of the mold and flash. Engine 20 was on a detail at Branthoover Cutoff and was on scene within minutes with Chief 20 with the Lillian Avenue command. The driver and Chief stretched a line to the garage and made a quick knockdown. At this time the call was held to Engine and Truck 20. Truck 20's crew provided manpower for overhaul and to put out a few remaining hot spots. The fire was caused by a burn barrel that was located too close to the garage. The garage suffered damage to the roof, fascia, and man door. Crews were out for a half hour. The Chief and Rescue 20 turned the home in Clover Ridge over to maintenance staff and quickly departed for Logans Ferry Road. Director 610 arrived first and reported smoke showing from the single family residence. Rescue 20 arrived quickly behind Director 610, established command, and stretched a line to the structure. Entry was made to the home and the fire was quickly knocked down. Additional crews checked for extension in the attic and basement with nothing found. Other crews setup for ventilation and put tarps down in the basement. The fire appears to be accidental at this time. Sun, 12 Jun 2011 13:46:43 GMT http://www.murrysvillefire.com/apps/public/news/newsView.cfm?News_ID=283 Chemical Reaction Causes Evacuation at Atria's http://www.murrysvillefire.com/apps/public/news/newsView.cfm?News_ID=282 At 21:33 hours, Station 20 was dispatched to 4869 William Penn Highway at Atria's Restaurant for an unknown chemical odor in the structure. The caller from the restaurant advised that an evacuation was in progress and that some employees were gagging from the fumes. Truck 20 arrived simultaneously with Engine 22, Rescue 64, and Special Operations 78, and Lieutenant 20-1 established the Atria's command. An interior crew led by Sergeant 20-1 was sent into the structure to determine if all employees and customers were safely evacuated (25 staff, 40 customers). Once it was determined the everyone had evacuated, air monitoring was done with no readings found. Chief 64 met with the kitchen staff who indicated that they had been using Lime Eliminator, bleach, and a pot and pan scrubbing solution and that those chemicals were mixed in a five gallon bucket. The interior crew found the three chemicals still in their original containers, made sure they were secure, and took them to Chief 64 so the active ingredients could be identified. The Lime Eliminator (phosphoric acid) was mixed with the bleach (sodium hypocholrite) and formed hydrogen chlorine gas, which is a poisonous gas. The interior crew thoroughly rinsed down the five gallon bucket, that had a small amount of liquid residue in it, and then washed down the floor and surrounding floor drains with approximately 200 gallons of water. The sink that was used by the employees was allowed to run for 15 minutes to clear any chemical residue from the traps. While the interior crew were rinsing the floors and floor drains, ventilation was established by using two PPV fans (one at Side A and one at Side D) as well as an exhaust fan in the entrance to the kitchen. This allowed the air from the restaurant to be ventilated through the kitchen and out of the rear of the structure. These fans ran for an hour to clear out any of the remaining fumes. A crew from Engine 22 checked the nearby stores for any odors and nothing was found. During this time, Murrysville Medic One setup a triage area to deal with the patients that had been exposed to the gas. Ambulances from Murrysville, Mutual Aid, Penn Township, and Oklahoma were used to transport six ALS patients and two BLS patients to area hospitals. Once Team 800 (Westmoreland County Hazardous Materials Response Team) officials were consulted by phone and a walk through by officers on scene, it was determined that the chemical residue had been thoroughly diluted and washed from the drains and that the building had been properly ventilated. Medic 613 and Support 610 were the first arriving units and advised of a vehicle into the building and over a wall into a landscaped area. Rescue 20 arrived on scene with Captain 20 establishing the Old William Penn command. The crew from Rescue 20 assessed the stability of the vehicle and the structure as well as checking on the condition of the sole occupant of the vehicle. With the possibility of structural damage and the need for structural support, SOC and Collapse 78 were put on standby. With a support column demolished and an exterior brick wall showing signs of deflection, SOC and Collapse 78 were moved to the scene to provide stabilization. The sole patient refused treatment and was aided in moving to a police vehicle where his information could safely be gathered. Meanwhile, other crews performed interior checks on the building for structural damage as well as monitoring for natural gas. Damage to the interior of the church was relatively minor considering the impact of the SUV on the building. Murrysville Police requested Wally's Transport to the scene to remove the vehicle from the landscaping bed. Crews from 78 shored up the front porch roof and covered the shattered windows in the entryway. The Building Inspector for the Municipality was called to the scene and confirmed that the building was safe for occupancy. All units cleared by 13:00 hours. Good job to all involved! Director 610 on scene with a heavy smoke condition in the basement with an evacuation of the staff and four legged patients. Chief 20 arrived and established the William Penn command. Truck 20 took the first due truck company position on side A of the structure and advanced a hose line into the structure. Engine 78 arrived next taking the first due engine company position and picking up the hydrant. The interior crew found an overpressurized boiler in the basement that was releasing large amounts of steam from a pressure relief valve. Power was shutoff at the boiler and a crew on the exterior secured the gas meters to the structure. The Animal Hospital will be relying on an alternative heating source until their boiler can be repaired or replaced. The scene was turned over to a representative from Dominion Peoples who placed the boiler out of service. During the incident, Murrysville Medic One provided medical and rehab services. Murrysville Police provided traffic control by closing down the slow lane of eastbound Route 22. Director 610 was the first to arrive on scene and reported a light haze of smoke issuing from the building with a moderate smoke condition on the interior. Truck 20 took the first due truck company position on Side A of the structure, and Assistant 20-2 established the Old William Penn command (later to be transferred to Assistant 20-1). The truck crew forced entry into the structure and advanced a hose line into the structure. The chauffer set the ladder for the incoming roof crew from Special Ops 78. The crew from Engine 22 set ground ladders and prepared for ventilation. Engine 9 provided a water supply to Truck 20 while their crew provided hands on the interior. Engine 78 and 64 secured the hydrant on Old William Penn. The Westmoreland County HazMat Team 800 staged a rapid intervention crew. Interior crews found a furnace on the second floor of the structure that had malfunctioned. The furnace was still running despite being at over 400 degrees. The PVC piping and insulation around the furnace had ignited causing a smoke condition in the structure. No major structural damage was sustained. The utility companies secured the electric and gas to the building. The alarm was scaled back to Truck and Rescue 20, Engine 9, and Special Ops 78 while the investigation was completed. The building was then turned back over to the owner and to a disaster restoration company. Rehab services and medical care were provided by Murrysville Medic One. Murrysville PD shut down Old William Penn Highway as well as one lane of Route 22 Westbound during the incident. Dir 610 arrived with smoke showing from the rear of the plaza near a storage room. T-20 quickly went enroute with crew of 4. T-20 arrived on B/C corner and went inservice with an 1 3/4" to rear stairwell. LT 20-1 placed the unit in service and established Franklin Plaza Command. Small fire load was located and extinguished in rear storage closet with possible extension to second floor and adjacent rooms. Asst 64-2 was assigned interior operations with negative extension found. Second floor and adjacent shops required extensive ventilation in which C-78 and T-236 crews handled with PPV. Due to sprinkler head activation Salvage 22 and 63 were placed into service for water removal. T-236 laddered side A and crews reported negative findings. The box assignment was held to 1 and 1. Crews remained on scene for approx 3 hours until power and gas was re-established. Attack 9 provided cover to District 1 while apparatus were on scene. At approx 1630 Command was terminated and all crews were released. Fire is currently being investigated by FM 20. Rescue 20 would go in service with a crew of 6 with Asst 202 with command. Director 610 would advise no injuries and no hazards with the vehicle overturned about 20 ft off the road. Crew would help with traffic control and wait for tow. All units cleared in 30 mins. Resuce 20 would go enroute with 6 and Chief and Asst 20 would arrive on scene with a single vehicle overturned with all occupants out of the vehicle. Rescue crew monitored hazards and waited for the tow to arrive to upright. Crews would return at 23:01 . Murrysville PD was first on scene and reported a working fire. A crew from Medic One updated that report to a 12X15 shed with exposures. Chief 20 and Rescue 20 arrived next. The Chief established the Heavens Lane Command and reported that the shed contained two - 250 gallon home heating oil tanks and was approximately 15 feet from the house and small barn. The crew from Rescue 20 pulled a three inch leader line and positioned it to the rear of the house. Water was provided in the form of a tanker shuttle using multiple engines and Tanker 64. With the leader line in place, two handlines were connected to start making a knockdown on the fire using class B foam since both oil tanks were on fire. The crew from Engine 22 pulled an additional handline to protect exposures using the secondary driveway while Special Operations 78 provided manpower to supplement the attack lines. Rescue 800 handled the runoff of home heating oil by using earthen breaks, absorbent pads, and booms. Rescue 87 handled the rapid intervention assignment. The shed was a complete loss but damage to the home and barn were prevented. The cause of the fire is undetermined. Crews were on scene for approximately three hours. Medic 615 called out on scene with one vehicle on its side over an embankment approximately 150 yards off the roadway wrapped around a tree. The lone occupant of the vehicle was heavily entrapped in the driver's seat. With the updated report of entrapment, Chief 20 had 911 activate Rescue 64 and Special Operations 78. With the School Road South command established, the crew from Rescue 20 began their way over the embankment to assess the situation and to determine the best course of extrication. The portable hydraulic unit with assorted Hurst tools, Junkyard Dog stabilization struts, and assorted cribbing were taken to the vehicle. By then, crews from 64 and 78 had arrived to assist bringing additional manpower and equipment. Due to the location of the vehicle on the hillside, it was secured by a winch from Rescue 20 and 64. The rescue technicians performed a reverse roof flap as well as carefully cutting the rear of the driver's seat away so that the patient could be removed via the rear window/B post area. Due to the patient's body position in the vehicle and the compartmentalization of the cab, the extrication was tedious and meticulous. During the extrication process, Director and Lieutenant 610 maintained patient care. Total time for the extrication was approximately one hour and twenty minutes. The patient was transported by an ALS unit from Medic One to Station 20 where a landing zone for a medical helicopter had been established by Rescue 9. The patient was then flown to a Pittsburgh trauma hospital for treatment of their injuries. Crews from 20, 64, 78, and Medic One worked extremely well together given a tough extrication in poor terrain. Tower Ladder 20 arrived to find a large sink hole across Meadowbrook Road with one SUV in the sink hole. The lone occupant of the SUV had safely made it from their vehicle. Captain 20 established the Meadowbrook command and requested crews from 64 and 78 to make their way into the single family home to control the utilities and monitor for hazardous levels of natural gas. Crews confirmed that all occupants of the home were safely out of the structure, and that the gas meter was turned off. A crew from the Municipal Authority of Westmoreland County (MAWC) arrived and turned off the water main. At this time, the tow was able to pull the SUV out of the sink hole. Unfortunately due to the location of the water line break as well as the downhill location of the residence, the house sustained major water damage on the first floor as well as the basement. Thermal Industries, a window manufacturer located in Murrysville, also sustained flooding damage as their facility is located below Meadowbrook Road. The crew from Engine 22 checked on the facility and helped the Thermal Industries staff locate floor drains. Thanks to Murrysville Public Works for providing road closures at both ends of Meadowbrook Road. The scene was turned over to the MAWC service crew and their risk management division. Chief 20 and Assistant 20-1 responded directly to the scene. Assistant 20-1 arrived to find one vehicle off the roadway, overturned, and leaking gasoline. A single patient was down in the front yard of the home and was being tended to by Murrysville Medic One. Crew from Rescue 20 controlled the leaking fluids from the vehicle and provided scene lighting for Murrysville Police. The patient was transported to a local hospital. The scene was turned over to Murrysville Police while they waited for the tow to make the vehicle recovery. All 20 units back in service by 01:45. Lieutenant 20-2 arrived on scene to find two vehicles, head-on type collision with one vehicle up on an embankment. All patients were to be out of the vehicles with at least one head injury. Chief 20 arrived and established the Old William Penn command. Rescue 20 arrived shortly thereafter and positioned in front of the accident to protect the scene. Rescue technicians cribbed the vehicles, controlled fluids, and cleaned off the roadway. Medic One treated and transported multiple patients to area hospitals. Crews out approximately 50 minutes. While a handful of members were standing on the pad after returning from the previous run, a loud sound of smashing metal and plastic was heard from the area in front of the station. Rescue 20 and Medic One pulled down the driveway to find a two vehicle, head-on type accident with injuries. Assistant 20-1 established the Sardis Road command and put the rescue crew to work. Rescue technicians provided patient care, blocked the roadway, cribbed the vehicles, controlled fluids, and cleaned off the roadway. One person was transported to an area hospital. Crews out approximately 45 minutes. Good teamwork at both accidents by the crews from Rescue 20, Murrysville Medic One, Murrysville Police, Wally's Transport, and McElhinney's Service. Director 610 called out on scene with smoke showing from a three story, middle of the row townhome. Engine 78 took the first due engine company position and stretched a 1-3/4" compressed air foam (CAF) attack line into the residence. Engine 236 picked up the hydrant on Sandune near Logans Ferry and laid into Engine 78. Truck 236 was next on scene and took the first due truck company position followed by Truck 20. The crew from Truck 236 threw ground ladders to sides A and C and set the stick for roof operations. The engine crew from 78 made a quick knock of the fire on Division 2 of the structure. While this happened, additional crews from Engine 236 and Truck 20 performed primary and secondary searches of the residence with both searches turning up negative. The crew from Truck 20 performed overhaul of Division 2 and opened windows for positive pressure ventilation. Additional crews from 64, 20, and 22 checked for extension in the neighboring townhomes. The neighboring townhomes were ventilated due to carbon monoxide. Team 800 provided rapid intervention services while Murrysville Medic One and Lower Kiski provided firefighter rehab. Command 20 released all units at 23:30 hours with the exception of Truck 20. The truck crew was on scene for an additional hour completing the fire investigation. Great job to everyone on scene! Paiano) EMS/Rehab: Director 610 (Gerano) While leaving the station for the structure fire, dispatch alerted Truck 20 for a second call in our area at Brookside Condominiums for an automatic fire alarm. Thanks to Stations 7, 9, 86, 87, and Monroeville 4 for handling the call for us. The cause of the alarm was a problem with the riser for the sprinkler system. The truck crew handled one additional call after returning from the structure fire - an automatic fire alarm at Cornerstone Ministries in District 2. The alarm was in the elevator shaft and was most likely caused by the power outage in the area (three phase power problem). All Station 20 units back in service at approximately 02:00 hours.
